What to Bring to Appointments
You must bring your insurance card or current Medicaid Card/document and identification. Payment is due at time of service rendered so please be prepared to pay with cash, check, Visa or MasterCard.

Financial Policy
Payment is required at time of service. All co-pays are due at time of service. If you do not have insurance you will be required to make payment in full on that day. We accept Visa, MasterCard, Cash and Checks. All checks must be preprinted with your information.

If payment cannot be made at time of service arrangements must be made with the billing department before making your appointment

There will be a $30 service charge on all returned checks.

The Parent or Guardian who brings the child in for their visit is responsible for payment at that time of service independent of what a divorce decree or other court documents may state.

Reimbursement must be made between the two parties, we will not intervene.

In order to accommodate the needs and requests of the patients, William A. Paruolo. M.D., P.A. has enrolled in numerous insurance programs. We are pleased to be able to provide this service to you, but it is impossible for us to keep track of all the individual requirements. Each plan has different stipulations regarding how often, where, and by whom medical services may be rendered. Even within the same insurance company, the plans differ with the type of contract you or your employer has negotiated.

Providing quality medical care for our patients is our primary concern. Please make every effort to become familiar with the benefits of your plan. We may recommend or render medical services that are not covered by your insurance plan. Should your insurance carrier require you to use a specific lab or other outside facility, it is your responsibility to inform the nurse. Failure to do so may result in charges to you which your insurance company does not cover. Most plans require that we receive authorization for referrals to a plan's list of specialists. These authorizations must be received well in advance of your appointment with the specialist. If a specialist is not on your "list" or if authorization is not received in advance, you will be required to pay out of pocket for the appointment. You will be billed for these services directly. Again, please make every effort to become familiar with the benefits of your plan.

Most insurance plans reimburse us a contractually agreed upon amount for our services. Should you have any past due outstanding balances, this will be collected before you are seen by the doctor, as well as co-payments that apply.

If there are any questions regarding a bill, please call and speak to our billing department. We are contracted by numerous insurance companies, if you are unsure whether we accept you’re insurance please call us with your information and we will let you know.
